    About the Role

    You will provide person-centred care coordination for clients living with complex aged and healthcare needs, whilst also utilising discipline specific skills to support each client to improve their health and wellbeing.



    This role includes conducting comprehensive holistic health assessments and discipline specific assessments, developing client centred care plans, coordinating health and social care needs and ensuring appropriate linkages with hospital and community services.

    • Undertake holistic comprehensive assessments and develop client centred care plans in the context of their health, functional, psychosocial, cultural, environmental, safety and economic needs.
    • Identify and access multidisciplinary and specialist supports to address specific client issues.
    • Monitor health outcomes, goal achievement and plan for the transition and exit from the program, ensuring appropriate long term community supports are in place on discharge.
    • Direct clinical provision of high quality and evidence-based discipline specific assessment, intervention, education and management.

    Classifications for this position will range from Grade 4A Registered Nurse or Grade 2 Allied Health

    Your Contribution

    • Current AHPRA registration if required for your discipline
    • Current Victorian Driver’s License
    • Experience working with aged clients living with chronic diseases and complex health and psychosocial conditions.
    • Current National criminal history check, or willing to obtain

    About Complex Care and Chronic Disease

    This position is under the Health Independence Program (HIP) and will be part of the HIP Complex Care and Chronic Disease Team. The key aim of the team is to improve health outcomes of people who either frequently present, or who are at risk of presenting to hospital. Clinical service delivery will focus predominantly on aged care and is undertaken in the clients’ own home and their local community.

    Complex Care and Chronic Disease currently consists of multidisciplinary teams based at SVHM Fitzroy and at North Richmond Community Health (NRCH). This position will be based at Fitzroy.
